Procedure for Lost Physical Certificates

When physical share certificates cannot be found or are damaged, shareholders cannot transact or convert their holdings into Demat without applying for duplicate issuance.

Under SEBI regulations, physical replacement paper certificates are no longer printed; instead, the company's RTA issues a Letter of Confirmation (LOC) in lieu of duplicate certificates, which is directly submitted to your Depository Participant (DP) for electronic credit.

Required Documentation

  • SEBI Form ISR-4: Request for issue of duplicate certificate / Letter of Confirmation.
  • Indemnity Bond (Form B): On non-judicial stamp paper of prescribed state stamp duty value.
  • Affidavit (Form A): Duly notarised affirming non-disposal and loss of original shares.
  • Police FIR / Non-Traceable Certificate: Stating folio number, certificate numbers, and distinctive numbers (if required based on valuation).
  • Newspaper Publication: Public notice in English and regional vernacular daily newspapers (for high-value folios).
  • Self-attested KYC documents (PAN, Aadhaar, Proof of Address) and Banker's signature attestation (ISR-2).
